Upload all the files in the '!upload' folder into the root of your forums directory.
Then login to your admin Panel and install the plugin.
If you want to add a button for your users, take a look at the ADDING_BUTTON.TXT file.

Righty, this one is a little more easy than the last version. Just copy and paste the URL of the video into the tag. For example:
